Simon's non-extendably shellable example

Description
Simon's example of a 2-dim simplicial complex which is shellable but not extendably.
Appears in Simon's 1994 paper in the appendix section. Having the same structure as nonextend.dat. That is, this example is contractible and its boundary consists of just one edge.
